Update Dry Eye Disease and Ocular Surface Inflammation

Inflammation has been included in the recent definitions of dry eye disease as a core mechanism. In this pathological process, ocular surface epithelia are both targets of the environmental and inflammatory stress and actors, through their ability to synthesize and release a wide range of inflammatory cytokines. Whether inflammation is a primary process causing the disease or a secondary event amplifying the ocular surface impairment is still debated. It is likely that the two mechanisms may explain different pathways to enter the vicious circle of dry eye disease and that inflammation rapidly plays a central role in sustaining the pathological state.
